Apicoectomy

When a Root Canal is not enough, an Apicoectomy can eliminate infection and preserve your natural tooth. Our board-certified oral and maxillofacial surgeons provide expert Apicoectomy care at two convenient Central New Jersey locations.

Apicoectomy

Overview

What is an Apicoectomy?

An Apicoectomy, sometimes called Root End Surgery, is a minor surgical procedure that removes the very tip of a tooth's root along with any surrounding infected tissue. Rather than extracting the tooth, it allows your oral and maxillofacial surgeon to address the problem directly at its source, preserving your natural smile and avoiding the need for a Dental Implant or other replacement. Most patients are surprised by how straightforward the experience is and how quickly they get back to normal.

When do you need an Apicoectomy?

You may need an Apicoectomy if a previous Root Canal has not fully resolved an infection, if imaging reveals a cyst or lesion at the root tip, or if retreating the Root Canal is not a viable option due to a post, crown, or complex root anatomy. Persistent pain or swelling near a tooth that has already been treated is often the first sign that something needs further attention. If your dentist or endodontist has referred you for an Apicoectomy in the Princeton Junction or Mercerville area, our surgeons are ready to help. We use advanced 3D Imaging and microsurgical techniques to evaluate your situation and recommend the most effective path forward.

Benefits of an Apicoectomy

  • Saves your natural tooth, avoiding extraction and replacement
  • Directly targets infection or inflammation at the root tip
  • Minimally invasive with a relatively straightforward recovery
  • Performed under Local Anesthesia with Sedation options available
  • Resolves persistent symptoms that Root Canal treatment alone could not
  • Preserves surrounding bone and tissue for long-term oral health

From prep to recovery, you’re in good hands.

What to expect

Before surgery

Your oral and maxillofacial surgeon will review your dental and medical history and examine your imaging to confirm that an Apicoectomy is the right approach for your situation. You will receive pre-operative instructions covering dietary restrictions, medications to pause, and what to arrange for the day of your procedure. If you have any anxiety about surgery, ask us about our sedation options at either of our Central New Jersey offices.

During surgery

Your surgeon will administer Local Anesthesia before making a small incision in the gum tissue near the affected tooth. The tip of the root and any infected tissue are carefully removed, and a small filling is placed to seal the root end. The incision is then closed with sutures. Most Apicoectomy procedures are completed in one to two hours.

After surgery

Some swelling, mild discomfort, and light bleeding in the first day or two are normal and expected. Your surgeon will provide detailed post-operative instructions covering pain management, diet, and activity restrictions. Most patients return to normal daily activity within a few days, and a follow-up visit allows your surgeon to confirm that the infection has resolved and healing is on track.
